Chronic disease management
Chronic diseases such as asthma, heart disease and diabetes can be debilitating for some. Our focus is to work with you to keep you as healthy as possible. We take an integrated care approach to managing illness which includes screenings, check-ups, monitoring and coordinating treatment, and patient education.
If you have a chronic medical condition, your doctor may suggest a GP Management Plan (GPMP), to formalize the appropriate actions that should be taken to optimize your care.
If you need treatment from two or more health professionals, your doctor may also put a Team Care Arrangement plan in place for you. This enables you to access further Medicare funding for extra health services you require.
We carry out spirometry, pulmonary function testing to see how your lungs are working. This is often used in the diagnosis of asthma, chronic obstructive pulmonary disease and other conditions that can affect your breathing. A $5 charge applies for the reusable tube used during testing.
